ETA: The second question does not apply to widely known phrases that a lot of people can be reasonably expected to know such as famous quotes, etc.

Son of ETA: I used the phrase "knee-jerk" in that question for a reason! I am aware that it is not a black-and-white issue and there are many circumstances on which your answer might depend. "knee-jerk" conclusion is not a "reasonable and thoughtful" one! :P
